  • Fill out Form No. (103) Notice of Enrollment / Termination of the Insured's Service.
  • Copy of:
    • Civil ID.
    • Nationality Certificate.
    • Birth certificate.
    • Employment contract or appointment decision.
  • Note: For lawyers, a copy of the membership ID of the Kuwait Bar Association and a “To Whom It May Concern” certificate from the Bar Association must be attached.
  • Fill out Form No. (103) Notice of Enrollment/Termination of the Insured's Service.
  • Copy of service termination decision.
  • Fill out the registration form for self-employed.
  • Copy of:
    • Civil ID.
    • Nationality Certificate.
    • Birth certificate.
    • Academic certificate indicating the latest academic qualification.
    • A license or proof of practicing business activity, taking into account upon submission the historical validity of the document.
    • The company's articles of incorporation (and its deeds of amendment, if any).
  • Notes: For lawyers, the Institution must be provided with the following:
    • A certificate from Kuwait Bar Association for practicing the profession, whether he/she is the owner of a law firm or partner of an office, indicating the date of the practice.
    • A certificate from the Civil Information in the name of the law firm.
    • The ID of the attorney.
  • Fill out a termination form for those registered under Chapter Five (and request proof of termination, as the case may be).
  • Copy of:
    • An extract from the Ministry of Commerce proving the license being canceled or waived.
    • The company's articles of incorporation (and its deeds of amendment, if any).
  • Note: For lawyers, a "non-practicing" certificate from Kuwait Bar Association must be provided.
  • Fill out Form No. (104) for employee abroad.
  • Copy of:
    • Nationality Certificate.
    • Birth certificate.
    • Civil ID.
    • Employment contract or salary certificate attested by the competent authorities in the country in which he/she works, and the Kuwaiti embassy or consulate in the country of the workplace.
  • Notes:
    • A copy of the passport can serve as substitute in the event that it is not possible to provide the nationality, birth certificate and civil ID to temporarily fulfill the registration, provided that the required copies are submitted later.
    • In the event that there is no embassy or consulate in the country of the workplace, it is sufficient for the official authorities in that country to certify the required documents.
    • In case the employer is in Kuwait and the workplace is outside the country, the required documents shall be certified by the Kuwaiti Ministry of Foreign Affairs.
    • Please be aware that the aforementioned form (104) is also used when terminating the contribution, and in this case it is necessary to attach a certificate of experience, or “To Whom It May Concern”, issued by the employer indicating the salary he receives upon termination of the contribution and certified by the aforementioned official authorities.
  • Letter of conjoining request from the current employer.
  • Copy of:
    • The Amiri Decree according to which Kuwaiti nationality was granted.
    • A certificate of the service period to be conjoined, indicating the end of service indemnity, if any (experience certificate).
    • A recent salary certificate from the employer, stating the basic salary and social allowance for the government sector, and the gross salary for other sectors.
  • A recent salary certificate from the employer.
  • A copy of the civil ID.
  • Submit the application through the employer.
  • Copy of:
    • Decision to grant leave.
    • Decision to proceed on work after the end of the leave or the decision to cut the leave.
    • A statement of the monthly salary scale during the leave period to be calculated (Including basic salary and social allowance for the government sector).
  • Submission of the application.
  • A recent salary certificate.
  • Submit the application by the applicant through the employer.
  • A recent salary certificate that includes the following according to the employment sector:
    • Government Sector: basic salary, social allowance, and children’s allowance.
    • Private / Oil Sector: Gross salary.
  • Submit the application through the employer.
  • A recent salary certificate indicating the allowances subject to supplementary insurance.
  • A certificate of the period of service certified by the official authorities for the period to be conjoined.
  • A recent salary certificate.
  • A letter and application form for a commutation during service No (105) from the employer.
  • Copy of:
    • Civil ID, Nationality Certificate, Birth Certificate.
    • Birth certificate of children who are included in the social allowance.
    • Certificate of proof of disability for disabled children included in the social allowance, if any.
    • A certificate of proof of disability and a valid disability card for the disabled person when applying for a commutation during service, if any.
    • Salary transfer certificate from the bank with the IBAN.
    • Appointment decision or employment contract.
    • A recent detailed salary certificate including all the deductions from the salary, issued within a month from the date of the borrowing application during the service.
    • A statement proving the days of absence and special leaves without pay.
    • Note:
    • In the event that a woman receives a social allowance for her children, a copy of the letter of approval of the Civil Service Bureau to grant her the social allowance for her children that she supports and receives a social allowance for them must be submitted.
  • Civil ID.
  • Certificate from the Public Authority for Manpower stating the date of registration.
  • Salary transfer certificate with IBAN.
  • Certificate from the Ministry of Commerce and Industry that there is no license.
  • Certificate from the Ministry of Social Affairs and Labor that no assistance is received.
  • In the event that alimony judgments are delivered against the applicant, a certificate from the Ministry of Justice stating the alimony due on him is required.
    • Note: In the event that the application is submitted through the attorney, a copy of the power of attorney and the attorney’s Civil ID must be submitted.

  • Filling out Chapter Five forms.

    • License cancellation certificate from Ministry of Commerce and Industry
    • License relinquishment certificate from Ministry of Commerce and Industry
    • Confirmation of withdrawal from the company
    • Confirmation of termination of the company’s contract
    • Death certificate (in case of termination for death)
  • Filling out the application form.
  • Letter of conjoining request from the current employer.
  • A certificate on the previous service periods prior to obtainment of Kuwaiti nationality indicating periods commencement and termination dates and reason.
  • A recent salary certificate from the employer.
    (Government sector: indicating the basic salary and social allowance)
    (Civil and oil sectors: indicating gross salary)
  • Filling out the application form.
  • A recent salary certificate from the employer.
    (Government sector: indicating the basic salary and social allowance)
    (Civil and oil sectors: indicating gross salary)
  • Filling out the forms for employees working abroad.
  • Copy of nationality certificate.
  • Copy of birth certificate.
  • Copy of civil ID.
  • A “To Whom It May Concern” certificate issued from the employer indicating the work relationship existing between the employer and the insured and the amount of the monthly salary received by the insured, provided that this certificate shall be authenticated by the competent authorities in the country where he is working and the embassy or consulate of Kuwait there.
  • Noting that the aforementioned form (104) is also used when the contribution is terminated, and in this case it is necessary to attach a “To Whom It May Concern” certificate issued by the employer, indicating the salary he receives upon termination, of the contribution and it shall be authenticated by the aforementioned official authorities.
